Our primary care locations have received recognition from the National Committee for Quality Assurance as Patient-Centered Medical Homes (PCMH). This care model focuses on putting the patients first. It helps create better relationships between patients and their healthcare teams.

Park Slope Family Health Center at º¬Ð߲ݴ«Ã½ Langone offers primary care and same-day sick visits for adults and children.

We also provide behavioral health and general and pediatric dentistry, and our medical staff is fluent in Russian and Spanish. As a patient at the Family Health Centers at º¬Ð߲ݴ«Ã½ Langone, you can also be referred to doctors from more than 30 adult and pediatric medical and surgical specialties as needed.

We are open Monday and Friday from 8:00AM to 5:00PM, Tuesday from 8:00AM to 6:00PM, and Wednesday and Thursday from 8:00AM to 8:00PM. We are closed Saturday and Sunday. Dental services are provided Monday through Friday from 9:00AM to 5:00PM.

After-Hours Medical Advice

If you have a non-urgent medical question outside of regular center hours, please call 718-832-5980. Assistance is available in multiple languages.

We are located near the B63 bus stop at 12th Street, the F line at Fourth Avenue, and M or R lines at 9th Street.
